Physical Information

ALL STUDENT ATHLETES GRADES 7-12 MUST HAVE A YEARLY SPORTS PHYSICAL OR A “FORM B” ON FILE PRIOR TO THEIR FIRST PRACTICE. PHYSICALS MUST BE ADMINISTERED ON OR AFTER APRIL 15TH IN ORDER TO BE EFFECTIVE FOR THE 2020-201 SCHOOL YEAR. NO STUDENT ATHLETE GRADES 7-12 WILL BE ALLOWED TO PRACTICE WITHOUT A CURRENT PHYSICAL.

Below is a statement from the NDHSAA:

Prior to participation (including practice) a student participating NDHSAA sanctioned athletic activities and representing his/her school must have on file with the superintendent, principal, athletic director or school nurse, an annual NDHSAA-approved Athletic Pre-Participation Health History Screening and Physical Examination form completed by a qualified health care professional (i.e. Doctor of Medicine of Osteopathy, Nurse Practitioner or Physician Assistant under the supervision of a physician); the Athletic Pre-Participation Health History Screening and Physical Examination is valid for one school year; a physical examination completed after April 15th is valid for athlete participation the following school year.

Activity Fees

Admission will be charged to all students if they do not have an activity ticket.  Students must either purchase activity tickets before the first home event or pay the regular admission charge.  Activity tickets are available from the office.

ATHLETES PLEASE DON’T FORGET TO PAY FOR YOUR ACTIVITY TICKET  BEFORE YOUR FIRST MATCH OR GAME!

 

Student Activity Ticket:

  • Elementary:  $20.00
  • 7th - 12th Grades:  $25.00

 

Regular Admission Charge:

  • Adults:  $6.00
  • Students K-12:  $4.00
  • Sports Tickets (16 adult admissions):  $75.00

 

Please be advised that there will be NO passes or activity tickets allowed for football games.  Due to the coop – Grant County/Flasher Storm, the joint boards have decided that admission will be charged to ALL patrons.
